Please Read Before Submitting Application:
We have discontinued accepting new Courtesy Membership applications as of July 15th, 2011.
Applications already in process are excluded, and will be afforded one full year of Trial Membership until July 21st, 2012.
Terms of New Membership Application:
If you are new to the field of Dental Legal Consulting check off "Beginner" in the online Membership Application provided below. Beginners with less than 1 year of experience will recieve $25 off of their membership. We will make every effort to assist you in taking the proper course work needed to do your Job well, and avoid costly mistakes that we have all made. You will also have access to our "Chat Blog" to learn from the experience of other members.
By clicking the submit button you are accepting our terms of membership. Due to several false applications, a Public Records check is required for all new requests. Our intention is to confirm your credentials, and to confirm that you actually do participate in Dental Legal Consulting or related fields. These services include Dental Forensics, Independent Medical Examinations in Dentistry, Dental Arbitrators, and Dental Related Legal Consulting (Professional, Scientific, or Technical, etc.).
A NON-REFUNDABLE $48 New Application Fee is required to process your application for review and verification of your credentials. You will recieve a password to make your Application Fee payment after intial screening of your entries. This step is intended to insure that the application was properly completed before formal submission.
If your Membership Application is Approved you will be given instructions via e-mail with instructions for submitting an online payment to cover your first year of Associate Membership ($125).
Deliberate falsification of information may lead to immediate removal from our C/V Referral Service,
and Posted Suspension from our organization.
As a member you are expected to abide by the AAODLC.org Code of Conduct.
